How to return your Rate Module
You should have recieved a postage paid return envelope in your
package. Simply place the module into the self addressed envelope
along with your customer number an mail to:

PLEASE NOTE:
You will be assesed a $175.00 charge (in additon to the purchase
price of the new rate module) for each rate module not returned
to Memphis within 15 days of receipt of the new module.
